I, the undersigned parent or legal guardian, acknowledge that boxing and combat sports training involve inherent risks of physical injury, including but not limited to bruises, cuts, sprains, fractures, concussions, and other serious injuries. I voluntarily consent to my minor child's participation and assume all such risks on their behalf.
In consideration of my minor child being permitted to participate in training activities at Off The Ropes Boxing ("the Gym"), located in Moreno Valley, California, I hereby release, waive, discharge, and covenant not to sue Off The Ropes Boxing, its owners, coaches, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my minor child during participation in any activity at the Gym.
I certify that my minor child is in good physical health and has no medical conditions that would prevent safe participation in boxing training. I agree to inform the coaching staff of any injuries, medical conditions, or physical limitations before training begins.
I agree that my minor child will follow all gym rules, coaching instructions, and safety protocols at all times. I understand that failure to comply may result in removal from the premises.
I grant Off The Ropes Boxing permission to use photographs or video footage of my minor child taken during training for promotional purposes, including social media and marketing materials, unless I provide written notice otherwise.
I authorize the Gym's staff to obtain emergency medical treatment for my minor child if I cannot be reached in a timely manner. I accept financial responsibility for any such treatment.
This waiver shall be governed by the laws of the State of California. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect. This waiver is binding upon my heirs, executors, administrators, and assigns.
By signing below, I confirm that I am the parent or legal guardian of the minor named above, that I have read and fully understand this waiver, and that I am signing this agreement voluntarily on behalf of my minor child.
